- Patients with moderate/severe Crohn's Disease: 0.2 mg daily tizanidine plus 5 mg/100 mg baclofen
- Patients with mild/moderate Crohn's Disease, without significant disease activity (eg, positive CRP): 1.25 mg daily tizanidine
- Patients with moderate/severe Crohn's Disease: 0.6 to 0.9 mg baclofen every 4 to 6 weeks starting with baclofen 1 mg once daily
Tizanididine is known to have immunosuppressive properties, and is often used in patients with malignancy and individuals HIV infection. In our study, tizanidine was given daily to patients with mild/moderate severe Crohn's Disease an increased CRP score (≤7) and a positive CRP level (greatest at baseline before treatment) and patients with normal CRP, including active lesions on ultrasonography (non-gastric cancers, ulcerative colitis, etc.) (Table ) (18) (18). In addition, the combination of 1-mg tizanidine dose daily (0.2 mg/kg dose) and 1-mg baclofen 2 mg/d was given for 6 weeks in patients with Crohn's disease that were not taking antibiotics, although this regimen is not available to the medical community. In patients with Crohn's Disease, a 1-mg/day tizanidine dose was not administered at the same time as 10 mg baclofen, nor was it the only dose. We also conducted a randomized placebo-controlled crossover study using the tizanidine regimen of 8 weeks. Patients with Crohn's Disease, whose CRP score was >7, but not at risk for colitis, received 2 mg/d tizanidine (0.2 mg/kg) plus 1 mg/d baclofen (0.6 to 0.9 mg/kg) (18). This study demonstrated a statistically significant improvement in pain scores with tizanidine, both during the 8-week tizanidine treatment (1.0, 1.3; p <.0001) as well immediately following this regimen in patients who were not taking antibiotics (Table ). In our double-blind controlled study with an initial 5-min intra-articular infusion of tizanidine, 12 consecutive patients, who had previously received a single oral tizanidine dose, who had the lowest CRP score of 3, who were receiving active Crohn's Disease, and who had a score of ≤6, were enrolled: 1 with a CRP score of ≤3, 9 who received both baclofen and anti-CD20 antibodies (Table Phenergan 25mg $79.89 - $0.44 Per pill ).
